An auditable screening process records who did what, to which record, when, and why — in order, without gaps, and without the ability to edit history after the fact.
Make the reason part of the action
If a status change can be saved without a reason, reasons will be missing. Requiring a rationale — and the failing criterion for a screen fail — at the moment of the decision costs seconds and makes later analysis possible.
Pin decisions to versions
A screening decision made under criteria version 1.0 should stay linked to 1.0 after an amendment. Flag affected records for review; don’t silently change them.
